I am a registered and participating member of St. Philip the Apostle Parish, and I affirm the following:

I am at least 16 years of age.

I am not a parent of the child.

I have already received the three Sacraments of Initiation: Baptism, Eucharist, and Confirmation.

I am in good standing with the Catholic Church and not living a public life contrary to the teachings of the Catholic Faith.

I am a practicing Catholic, and I participate in the Mass each Sunday.

I actively strive to live out my commitment to Jesus Christ and to the community life of the Church by my loving response to those with whom I come in contact daily.

I understand and accept the responsibility which I undertake as a sponsor. I intend to support the person I am sponsoring by my prayer, good example, and efforts to live a Christian life that reflects the faith and teaching of the Catholic church.
